Since 1980, Palmerston has been a leader in AOD support across WA, helping people improve their wellbeing. We offer specialised services in alcohol and other drug treatment, mental health, and wellbeing, through counselling, group support, residential rehabilitation, and education—reaching communities across the Perth metro, Peel, South West, and Great Southern regions.

About the Role

📌This role forms part of a multidisciplinary team delivering the Active Recovery Team (ART) in Armadale, a collaborative program between East Metropolitan Health Service ,  Armadale Kalamunda Group, the Mental Illness Fellowship of WA, and Carers WA.

The ART program provides up to 90 days of tailored, recovery-focused support to individuals following an acute or crisis episode of mental ill-health. Based at the Orchard Avenue Centre , you will manage a caseload of clients, delivering therapeutic counselling and facilitating access to community services and programs.

In this role, you will play a vital part in supporting individuals on their recovery journey. Responsibilities will include:

  • Contributing to ongoing collaborative assessments that consider each client’s needs, goals, and preferences.
  • Assisting in the development of individualised support plans that are meaningful and relevant to each client’s recovery.
  • Actively participating in multidisciplinary intake meetings and case reviews to ensure coordinated and holistic care.
  • Managing a caseload of clients, providing counselling, education, and relevant information tailored to their needs.
  • Working with clients to develop transition plans as they exit the ART program, including referrals to ongoing supports and services.
  • Maintaining accurate, up-to-date client records in line with organisational and program requirements.
  • Contributing to program reporting and evaluation through written reports and participation in review processes.

This role is classified at Level 4 under the Social, Community, Home Care and Disability Services Industry Award 2010.

What We’re Looking For

You’re more than your qualifications—you’re someone who brings heart, empathy, professionalism and a strong commitment to supporting clients in improving their wellbeing. The ideal candidate will meet the following selection criteria:

  • Tertiary (or equivalent) qualifications in social, behavioural or health sciences or equivalent experience.
  • Right to work in Australia.
  • Experience working with people experiencing mental health and alcohol and other drug challenges.
  • Understanding of recovery philosophy and experience in recovery planning.
  • Demonstrated assessment skills and well-developed case management skills.
  • A valid driver’s license and reliable vehicle is essential to this position.
