New Street Outreach Service in the Great Southern - Palmerston Association
Palmerston, in partnership with Pioneer Health, has launched a new Street Outreach service to support people experiencing homelessness in the Great Southern.
Funded by the Australian Government’s After Hours Primary Health Care – Homelessness Access Program (AHPHC-HAP), the service offers on-the-ground clinical care, harm reduction education and needle exchange as well as Psychosocial support and food provision.
By reaching people where they are, the initiative aims to break down barriers, build trust, and connect vulnerable members of the community with ongoing care and support.
“People experiencing homelessness often struggle to access even the most basic health services,” says Tina Edward-Pope, Palmerston General Manager Regions.
“This outreach program is about taking care to the streets and connecting individuals with the broader support systems that can help them live healthier lives.”
